Accepted Insurances

Angela does not take insurance but provides superbills to allow for individual reimbursement using out-of-network benefits. Courtney accepts the following insurances: Cigna and Anthem. It is your responsibility to know and understand the specific benefits of your insurance plan.

If you are out of network or do not have insurance, we offer a private pay option for individual counseling services. Your insurance will likely cover a portion of services using your out of network benefits.

Rebellious Wellness is proud to offer individualized counseling through telehealth. We also provide family support and assist with coordinating with and or building your clinical team.

Steps to determine out of network benefits

Call the member services number located on the back of your insurance card
Write down the representatives name, call reference number and all information that is provided to you
Refer to the following questions listed but not limited:
"Does my plan cover out of network outpatient therapy? The CPT codes for the services are 90834 or 90837. The place of service is 02 for virtual/telehealth sessions. Will any other codes need to be listed?"
If the answer is no, ask about a single case agreement due to the limited number of providers in the area that specialize with eating disorder treatment.
If the answer is yes, continue with the following questions:
"Are there any restrictions for what is covered? Is there a limited of sessions per year covered or is it unlimited? Is my plan limited to 40-minute or 50-minute sessions?" Make sure to inquire about when the year begins or ends with your plan.
"Do I need a physician referral?"
"What is the process for submitting a superbill reimbursement?"
